Individual care for those in need of care desired

2030 in Germany is estimated to face 100 working-age people, such as 50 people of retirement age. Most of these old people want to stay in their usual environment for as long as possible. For this they need individual support. Here, self-employed geriatric nurses help: They do not work at fixed times in the nursing home, but look after, look after and advise elderly people in need of help at home.

And it is precisely in this area that many new jobs are created - in the service sector, especially in health and care. A service provider of Company advises on the health of its employees, desperately looking for employees. In the midst of the crisis, 50.000 created additional jobs in the healthcare industry - and, like industrial production, they can not be outsourced abroad.

More old, less young

While the age structure in Germany beginning of the 20. In the 20th century, it was still largely that of a classic age pyramid. Today, the most populated vintages are found in an ever-increasing age. The reasons for this are the lower birth rate and the better health care, which is associated with a higher life expectancy.

The old-age quotient, that is the ratio of persons of retirement age to 100 persons of working age, was 2005 approx. 32. In the year 2030, as science predicts, it will be around 50 or 52, in 2050 60 and 64 respectively. The immigration of foreigners weakens this development, as migrants are often people of young age and their higher birth rate plays a role. However, earlier migratory movements have now largely neutralized.

A question of individual health

Unfortunately, older people and longer lives do not always mean that they enjoy the best of health. The health problems are of different nature: They range from minor restrictions of mobility over mental impairments to complete immobility. An elderly or nursing home with a rigid daily routine is usually the last option for many affected and relatives. Most participants wish that older people can stay in their usual environment as long as possible. And for this they need an individual, tailored to their personal needs.

This is where self-employed nurses continue to help: For this does not work at rigid times in the nursing home, but looks after various clients on an outpatient basis in their own home. nurture, look after and advise needy elderly people. The goal here is always to enable as far as possible, an independent lifestyle of the cared for and to receive. Usually, a private carer visits several clients a day, but it is also possible, of course, to only take care of one or a few patients a week. The extent to which a client is cared for, of course, depends on what price he is willing to pay.

What ambulant carers do

Depending on the entrepreneurial Objective Outpatient nursing staff either only provide basic care and housekeeping services or also provide medical services that are delegated by the doctor. Above all, however, they support older people, for example with everyday activities. This includes personal hygiene, eating or getting dressed. They talk to them about personal and social matters, accompany them, for example, when they go to the authorities or visit the doctor, have an activating effect on them and, for example, instruct them in health-promoting exercises or active leisure time activities.

They work together with family members and volunteers and also teach them in nursing techniques, which is especially important in home care. In nursing care and rehabilitation, geriatric nurses also perform therapeutic and medical nursing tasks, such as changing associations, performing rinses and administering medication under medical supervision. They help in emergencies and life-threatening situations, accompany the dying and provide for the deceased. In addition, they carry out organizational and administrative activities, eg writing care reports.

Offer additional options and services

They already have many larger care services on offer: optional and other services that clients can book in addition to care, for example arranging warm lunches such as “rolling lunch” or “meals on wheels”. You can also do this by teaming up with other experts and offering various modules: For example hairdressing and cosmetic services for the elderly, special sports and fitness offers tailored to the elderly and the sick, or massages. The service package could also include such simple things as taking care of plants or pets in the event of a hospital stay, accompanying them to the doctor or shopping or accompanying them on excursions or even trips.

Working as a self-employed outpatient nurse requires a certain amount of flexibility, because the caregiver is much more of a caregiver for his clients than, for example, in a nursing home. Therefore, it is necessary to be even more responsive to the needs of its customers and not to do them the necessary work, but to give them not only the necessary help and human attention.

How is outpatient care financed?

Outpatient care is financed by the benefits of long-term care insurance and - if this is not sufficient - by the social welfare office or self-paying patients or their relatives. Nursing and housekeeping activities within the framework of the nursing level are based on SGB XI and SGB XII (care help) and are performed in so-called service complexes, which are largely uniform.

Nevertheless, there are smaller differences in the individual federal states. For example, care services in Berlin can demand a travel allowance from their customers, whereas this is not possible in the state of Saxony. Each module is assigned a score. The amount of the remuneration per point is individually negotiated by the nursing care funds and social assistance institutions with the nursing service or an association.

Treatment care services, which must be prescribed by the doctor, are governed by SGB V and, depending on the health insurance, are reimbursed in a lump sum for each service (service charge + service charge) or as a lump sum for a service from a specific module. If support is needed after a stay in hospital, primary care and home help services can be provided through a doctor's prescription.

Prerequisites, education and training

Geriatric care is a nationwide uniformly regulated education, the school part of which is carried out at vocational schools for elderly care and their practical part in nursing homes. The training lasts 3 years. Model tests in which a further module can be acquired through an advanced module provide for a total training period of 3,5 years. Later, you can qualify further with a degree in nursing management or nursing education.

Prerequisite for this is a middle education degree or another completed ten-year school education, which extends the Hauptschulabschluss. Pupils with a secondary school diploma can also be enrolled if they also have a completed vocational training of at least two years or if they have submitted their license as nursing aides or if they have completed a successfully completed state-regulated training of at least one year in the nursing assistant or geriatric nursing assistant. Before starting training, the health suitability for the profession must be proven by a medical certificate.

Formalities in starting a business and opportunities

As a geriatric nurse you are working freelance. This means you only need a tax number at the tax office. A business registration is not required. However, as a self-employed caregiver, you must join a national framework agreement, conclude a health care contract and agree on a compensation agreement with the national health insurance associations and the associations of the long-term care insurance associations at the state level.

Elderly carers are urgently needed and the need will continue to grow in the future. The entrepreneurial activity can be expanded: You can start as a single caregiver to find out more about a Network develop and finally, as a long-term perspective, open your own nursing service with several employees. Specialization is also possible, because there are major differences in terms of the range of care services offered, especially when it comes to medical services. For example, some nurses are increasingly focused on intensive care or oncological care in the home. With such a special service you stand out from other caregivers.
